What is Premenstrual Dysphoric Disorder?
Premenstrual Dysphoric Disorder (PMDD) is characterized by significant mood swings, irritability, depression mood and/or anxiety in the week before you get your period. Simply, it is a more severe form of premenstrual syndrome (PMS) with more severe emotional symptoms and a significant impact on your functioning at home, at work and/or in relationships.
Symptoms of Premenstrual Dysphoric Disorder
Five or more of the following symptoms in the week before your period:
- Mood swings, sudden crying spells and increased sensitivity
- Irritability, anger outbursts, and more interpersonal conflicts
- Depressed mood or feelings of hopelessness
- Anxiety, tension and feeling of being “on edge”
- Lack of interest in things you usually enjoy
- Difficulty concentrating
- Low energy and fatigue
- Changes in appetite and overeating
- Sleeping too much or not enough
- Feeling overwhelmed or out of control
- Physical discomfort
- Subjective feeling of bloating or weight gain
Cognitive Behavioral Model of Premenstrual Dysphoric Disorder
The underlying causes of PMDD are unclear but it seems to be related to hormonal changes during your menstrual cycle. Women with preexisting anxiety and depression are also more likely to have PMDD. In the cognitive behavioral model, depression and anxiety are thought to be maintained by negative and often unrealistic beliefs about yourself, others and the world. This type of thinking may lead you to refrain from healthy and rewarding activities, including exercise, spending time outside your home, and accepting help from support system, which then further increases feelings of depression and negative thinking.
Cognitive Behavior Treatment of Premenstrual Dysphoric Disorder
Cognitive Behavior Therapy (CBT) helps women struggling with PMDD to increase positive and fulfilling experiences, while also modifying negative and unrealistic thoughts and interpretations about themselves that make PMDD symptoms worse. You will be guided in monitoring your mood and the accompanying thoughts and behaviors, which will help you understand your cycles and how negative thinking exacerbates depressed feelings leads to unhealthy coping. Your therapist will help you to evaluate negative thoughts and develop more realistic and helpful ways to think about yourself and others. Your therapist will also partner with your doctor to help determine if medication may lessen your symptoms.
